Sharing a snippet that cut hallucinations noticeably in my eval set.
When you are not at least 80% confident in a factual claim, say so explicitly and state what you would need to verify it. A calibrated "I am not sure" is a correct answer; a confident wrong answer is the worst outcome.
Works across Claude and GPT. Curious if others have a tighter version.
